Pre-Self Determination ~ Transition Series

This program will provide you with information about the adult service delivery system, how to access services, what benefit & entitlements are available and current regulations.  You will hear directly from High School Graduates about their experiences with the services system.  Through guided activities, you will begin to develop your own person centered plan to assist you through your transition from high school to the adult service system.  This program is for individuals and their families beginning the transition process from school to the adult world.
